Assessment & Evaluation ● The Partner District Liaison and Director of Special Education in the fall and spring of each school year will meet to assess and determine metrics of ongoing collaboration and mutual responsibilities. ● Action steps are mutually agreed upon and monitored throughout the school year ● Additional stakeholders (leadership, staff, students and community) should be considered throughout the year to provide input in the progress of the partnership, as applicable. 8

Action Steps 11 Action Steps to Strengthen the Partnership Between Clayton School District and Special School District: 1. Redesign the Transition Process:Revamp the transition process for key educational milestones, including Pre-K to Kindergarten, Grade 5 to Grade 6, Grade 8 to Grade 9, and Grade 12 to post-secondary pathways, ensuring smooth transitions for students with special needs. Measured by parent-facing engagement survey results, IEP participation surveys, and feedback from staff and families. 2. Align Resources with Goals: Ensure that academic and social-emotional resources and experts from both the School District of Clayton and the Special School District are available to support students with special needs, parents, and community members. This will be achieved by offering joint presentations and professional development opportunities.Measured by the increase in the number of joint presentations, professional development sessions, consultations between district coaches, Effective Practice Specialists, facilitators, and assistive technology referrals.

Increase Collaborative Walkthroughs: Conduct more frequent joint walkthroughs by principals and directors to align classroom universal supports, review curriculum and instructional practices, and provide staff with calibrated feedback that reflects Clayton’s academic and social-emotional priorities for students in special education settings. Measured by quarterly walkthrough feedback forms and notes from directors and principals. 4. Expand Two-way Communication Continue to collect feedback from stakeholders and align communication channels to the preferences and needs of our families. Measured by quarterly walkthrough feedback forms and notes from directors and principals.
