No applicant was present on behalf of the CUP. PC- ARB discussed the CUP. Bob Denlow made a motion to recommend approval of the CUP to the Board of Alderman as submitted. 4. 123 North Forsyth Boulevard – Site Plan Review– New Single-Family Consideration of a request by Toni Kaufer, Applicant/Owner, for Site Plan Review of a new single-family home. Ryan Helle provided a summary of the staff report with a recommendation to approve with the following conditions: 1) The applicant shall vertical mulch around the root structure of the street trees to alleviate any compression/consolidation caused by equipment running through the critical root zone 2) The applicant shall provide a percolation test report conducted by a qualified geotechnical engineer. The report must assess the performance of the proposed drywell based on the anticipated impact of a 15-year, 20-minute storm event. Should the results indicate that the proposed drywell is insufficient to accommodate the stormwater runoff generated by this event, the applicant shall be required to upsize the drywell accordingly to ensure adequate performance and compliance with applicable stormwater management standards. 3) To ensure the future maintenance and operation of the dry-well drive, the applicant shall record a deed restriction noting the location and maintenance requirements of the dry-well system and shall submit proof of the recording prior to issuance of a Building Permit.
